About Family Assistance Ministries (FAM): FAM is an interfaith non-profit organization whose mission is to assist those in need in Orange County with resources for food, shelter, and personalized supportive counsel and aid; helping clients bridge the gap from dependency to self-sufficiency.
Position Summary: The Case Manager provides one-on-one comprehensive case management to support at-risk, unhoused individuals, families, and persons in crisis from dependency to self-sufficiency.
Essential Functions & Responsibilities
Ensure comprehensive case management is provided including intake assessments, evaluating client’s needs, developing housing sustainability plans, and monitoring progress.
Maintain a caseload size that is determined based on the program’s needs.
Assist and refer clients to community resources, following up with weekly goals and completion of action steps.
Provide mobile advocacy case management as needed.
Link clients to resources such as Cal Fresh, General Relief, Cal Aim, State and Federal disability programs.
On an as needed basis, coordinate and meet with clients off-site to complete case management forms and build rapport with clients.
Perform crisis intervention, trauma-informed care, and motivational techniques to coach clients with any issues that are adversely affecting their lives through comprehensive case management best practices.
Collect required documentation for client aid requests and assemble client aid packets for review and approval.
Protect and maintain confidential case files for clients.
Collect and input data into FAM’s database, HMIS, and Cal Optima Connect within 72 business hours.
Collaborate with the Case Management Team by attending weekly meetings for case conferencing, discuss team and agency updates, and create new strategies to meet program needs.
Collaborate with clients to set achievable goals, such as accessing housing, medical care, mental health services, employment, and other necessary resources.
Maintain accurate and up to date case notes, progress reports, and documentation of services provided.
Provide crisis intervention and support to clients facing immediate challenges.
Meet with Housing Sustainability Manager on a weekly basis to strategize and review client cases to ensure best practices are utilized and implemented in alignment with the team and organizational goals and initiatives.
Report all suspected child, elder, dependent adult abuse to the appropriate agency as mandated by law.
Participate in regular team meetings and training sessions, as needed.
Other duties may be assigned.
Education and/or Experience Required
Bachelor’s degree is highly desirable or equivalent combination of training and experience.
Experience working with at-risk, unhoused individuals, families, and persons in crisis is highly desirable.
Experience with Housing First Principles and Trauma-informed Care are highly desirable
Proficient in Microsoft (Word, Excel, PowerPoint).
Experience in case management software highly desired.
Knowledge of homelessness-related issues, local resources, and social service systems.
Current valid CA driver's license and insurance.
Skills & Abilities
Ability to work with clients, collaborate effectively with the team, cross-functional teams, and with partner agencies.
Ability to exercise sound judgment with minimal direction/guidance.
Ability to prioritize tasks and exercise exceptional time management skills to meet program goals.
Demonstrate exceptional communication and customer service skills.
Ability to work in a highly dynamic environment is strongly preferred with interruptions and deadlines is strongly preferred.
Ability to drive to off-site locations for appointments, as needed.
Empathy, patience, and cultural competence when working with diverse populations.
Ability to maintain confidentiality and adherence to ethical standards in all interactions.
Working Conditions
This role may require up to 40% driving and local travel within Orange County to reach clients in different locations.
Physical demands may include walking, standing, and lifting during community activities.
May require at times to support community activities, events, and involve lifting up to 25 pounds.

Job Types: Full-time
Pay Range: $24 - $27 per hour.
